5c Diagnosis Double aortic arch (DAA) Discussion The DAA is the most common symptomatic vascular ring. Although esophageal compression may produce symptoms, respiratory findings, including distress, wheezing, or stridor, predominate. On plain-films, the DAA will demonstrate a right arch and generalized overaeration. Displacement or indentations on the trachea may be seen, or there may be complete loss of the tracheal air column on frontal projection, as in this case. The esophagram, however, is more valuable as the DAA produces a characteristic reverse S indentation on the esophagus.
Most cases present before the age of 2 years. A variety of presenting symptoms can occur, including neonatal bowel obstruction, intussusception, palpable mass, vomiting, and abdominal pain. Duplications may contain ectopic gastric or pancreatic mucosa that, when present, can induce intractable abdominal pain, GI bleeding, anemia, and elevated amylase levels. Plain-film findings may include a bowel obstruction or intussusception. Sonographically, a well-defined, spherical or tubular, fluid-filled mass with the characteristic bowel wall signature of hyperechoic inner lining of mucosa and hypoechoic rim of smooth muscle is specific for duplication.
